I have been experiencing an issue with the alerts feature on my iPhone 3GS running iOS 5.1.1.
I use a blank template task in which I have included 95 alarms programmed for certain time increments on certain days in relation to the task's due date.
I duplicate my blank template and rename it to create new recurring tasks using the blank template's alarms.
When they work properly both my recurring tasks and the blank template test task alarms go off successfully, with the task alarm sounding and information being displayed in the Notification Center and on the Lock Screen.

The issue is that, with more and more frequency, the alarms do not occur when they are programmed to. There is no alarm sound or notification information displayed in the Notification Center or the Lock Screen.

Other information that may be pertinent:
I have confirmed that I have 2Do "On" in the Notification Center, including Sounds, Badge App Icon, and View in Lock Screen.
If I have multiple tasks each with a sound alarm to initiate at 3:00 PM, at times the alarms for all three tasks occur properly as scheduled and other times only one alarm for one task does.
Alerts for newly tasks created by duplicating my blank task template will not go off on my phone when they are scheduled to, yet occasionally after I have synced via 2Do Sync Helper the alerts will then begin going off on my phone as scheduled. This is not always the case and I cannot determine a pattern as to when this does and does not work.
After syncing via 2Do Sync Helper with my computer, the alarms for a given task will properly execute on the computer via iCal, but the alarms for those same tasks do not execute on my iPhone.
This leads me to believe the issue is restricted to the app's execution on my phone, and not creation or properties of the tasks themselves within the app.

I tried to be as thorough as possible in describing the particulars of my alarm issues, hopefully this has been enough to go by. Inasmuch as it is psychologically possible, I love this app. That's probably why being spurned by my 2Do alarms in such a fashion is so disappointingly frustrating.
